Accessing Disability Services

 

To access services through Disability Services please follow the following steps.
1. Register with Disability Services
Call our office at (478) 445-4233 or stop by our office at 133 Maxwell Student Union.

2. Complete an Intake Form
We will provide you with an Intake Form that should be completed and mailed or faxed to us at (478) 445-1287 or delivered to us in person.   Our mailing address is:

3. Provide Disability Documentation
After the Intake Form has been completed and returned to us, we will give you the GCSU Documentation Guidelines for your specific disability(ies).

You should then provide us with documentation from an appropriate licensed professional that follows the GCSU Documentation Guidelines and documents your impairment(s). If documentation of disability does not exist, we can assist with seeking licensed professionals who can provide acceptable documentation.

Along with the documentation you can also include any records of accommodations/services provided to you in previous academic settings (e.g. IEP, SOP, Section 504 Plans, or letters from other collegiate institutions.)

4. Schedule an appointment
Once we receive your documentation, we will review all documentation that you have submitted.

After review, we will schedule an appointment with you to discuss your results.

If the documentation does not meet criteria, then we will let you know the areas where your documentation is lacking pertinent information. It is your responsibility to contact the medical professional, obtain the information and then make sure we receive any additional information to add to the original documentation submitted.

If the documentation does meet criteria, then we will discuss with you any accommodation (s) for which you qualify, as well as other services that may be available to you.

If you are granted Academic Accommodations, we will prepare an Accommodation Letter for you. Once you have received this letter, you should make appointments with all of your professors to discuss implementation of your accommodation(s) and to give them the letter from our office.
