IMMUNIZATION REQUIREMENTS
Georgia College & State University and The University System of Georgia require immunization documents from every newly admitted and readmitted student. Georgia College & State University will forward an immunization certificate form to each newly admitted and readmitted student. This form must be completed and returned to the Office of the Registrar, CBX 069, Milledgeville, GA 31061 for evaluation prior to enrollment.
