Required Forms to Participate

The following forms must be complete, sign, and emailed to milly48filmfestival@gmail.com upon submitting your film to the festival portal.

Participation Form

Please complete one form for everyone on a film team: Team Leader, cast, and crew. Please read it before you sign and ask questions first.
